  • Education: 2 Year / Associate Degree Required. 4 Year/Bachelor's degree preferred. Graduate of an accredited school of nursing required.
  • Experience: Minimum three years' experience in hospice or home health care required. Experience in direct care of patients with life limiting illnesses strongly preferred. Related experience include oncology; family medicine; internal medicine; geriatrics, medical surgical, and/or case management preferred.
  • Licensure/Certification: Appropriate state RN licensure required. Current Basic Life Support for Healthcare Provider (BLS) Required. CHPN/CHPCA Preferred.
  • Additional skills required : Ability to provide direct patient care, demonstrate excellent nursing skills, judgment, and understanding of Hospice philosophy. Excellent verbal and written communication skills, and problem-solving skills. Ability to prioritize, organize and handle varying and unpredictable situation such as emergencies or crisis situations. Ability to interpret information to identify each patient’s requirements for care relative to his/her age specific needs. Must be flexible, adaptable to change and able to apply problem-solving skills and critical thinking skills. Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees. Must have knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of infusion and enteral therapies. Must have computer experience and have reliable transportation to and from all work related assignments and activities.
    Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair. Ability to work in environment containing distractions and interruptions such as noise and telephone calls. Must be available for after hours call on rotational basis, as assigned.
