5 West Intermediate Critical Care Unit is an exciting 20 bed unit that specializes in stepdown care. An experienced RN is preferred but we are able to train on the job. Someone who is inquisitive and detail oriented are key skill we look for in a potential team member. Another asset we look for in perspective team members are those who are team players, outgoing, and those who are able to function mostly self-supported. Our team utilizes an interdisciplinary approach with collaboration from Healthcare Providers, Respiratory Services, Pharmacy, Physical and Occupational Therapies, and Nursing support. We serve a population that includes but is not limited to sepsis, diabetes, various degrees of renal failure, congestive heart failure, chronic obstructive pulmonary disease with non-invasive respiratory support, stable tracheostomy tubes not requiring mechanical ventilation, and those needing a vasoactive medication. Our preferred ratio is 3:1 but we do flex to 4:1. Essential Functions
• Assessment: Demonstrates refined assessment skills to collaboratively discover the patient's story,
integrating the following elements: subjective data, objective data, physical picture, psychosocial
components, personal preferences and norms, and learning needs and preferences. Must be able to
utilize an ongoing process to interpret data and recognize patterns and trends to determine both
immediate and future care needs.
• Planning: Dialogues with the patient/chosen support system to identify what is important to them
and incorporate his/her preferences and norms into the individualized care plan. Engages the
interdisciplinary care team in planning individualized care. Identifies relevant resources based on
necessity and availability. Determines immediate needs and establishes realistic short term and long
term goals based on desired outcomes. Prioritizes interventions to advance the patient toward
optimal health or support a peaceful death. Constantly considers ongoing modification of care plan
based on patient's needs, capabilities, and condition.
• Implementation of Interdisciplinary Plan of Care: Acts with patient advocacy at the core of nursing
care when implementing the interdisciplinary plan of care. Collaborates with healthcare team to
educate patient and their support system. Analyzes, modifies, and implements strategies to progress
the patient through their plan of care. Ensures completion of nursing interventions, medical orders,
and other related therapies. Possesses the ability to prioritize job duties according to patients' needs
and changing acuity levels. Administers and evaluates medication therapies, performs patient
transfers and transports patients via wheelchair, hospital bed, or stretcher to other departments.
Ability to lift and carry various equipment including portable cardiac monitors, bedside commodes,
etc. Ability to provide routine nursing care such as start IVs, perform dressing changes, etc. for
multiple patients in a timely manner. Ability to perform in critical situations including administering
CPR.
• Evaluation: Continuously assesses patient's progress toward goals and outcomes. Seeks input from
patient and their chosen support system regarding progress toward goals . Analyzes and modifies plan
of care with patient and interdisciplinary team as indicated to achieve desired outcomes.
• Communication and Documentation: Communicates individualized plan of care and patient's
progress toward goals to all members of the care team including the patient and his/her chosen
support system. Consistently utilizes appropriate communication tools such as Situation, Background,
Assessment, Recommendation, and Question (SBARQ) when communicating within the
interdisciplinary team and when handing off patient care. Documents appropriate information in a
timely manner.
• Delegation: Appropriately delegates to ensure the implementation of the plan of care while ensuring
patient safety, considering the nature of the task, competence of the delegate, and the patient's
response. Demonstrates commitment to quality patient care by holding self and others accountable
for optimal role performance.
• Performance Accountability/Evidence-Based Practice: Demonstrates independence in the
foundation of nursing care practice within the specialty. Practice according to your state's Nurse
Practice Act, ANA Code of Ethics and ANA Bill of Rights. Seeks involvement in Shared Governance
through council participation and/or by staying current with council activities. Identifies opportunities
for process improvement, incorporates evidence-based practice and research. Engage in self-reflection
and develop personal goals to enhance knowledge, obtain professional certification or pursue
academic degree as strategies to promote professional growth. Integrate gained knowledge into
practice. Maintain professional portfolio.
• Teamwork: Develops and maintains positive relationships with team members/internal customers.
Consistently demonstrates a willingness to adjust activities to accommodate the needs of team
members/internal customers. Develops team members through mentoring, precepting, orientation,
and providing feedback. Resolves conflicts independently, as appropriate. Accepts responsibility for
actions and responds positively upon feedback from others.
• Critical Care Specific: Hemodynamic monitoring, ventilator management, sedation and delirium
management, continuous renal replacement therapy, hypothermia management, vasoactive
medication management, cardiac rhythm interpretation and management, management of code blue,
infection prevention management related to central lines, ventilators, foley catheters and pressure
ulcers. Management of patient experiencing insults, neurological, pulmonary, cardiovascular,
immunological, renal, and gastrointestinal. Management of patients experiencing multi-organ
dysfunction related to sepsis/septic shock. May serves as a mentor/coach, a code blue leader and/or
RRT responder.
