The Care Connections Clinical Quality Coordinator works collaboratively with all disciplines within Care Connections, NHMG Payor Initiatives and across NH as well as any third-party clients to ensure safety, best practice, and high-quality standards of care are maintained across the continuum. Evaluate and analyze the effectiveness of Care Connections and NHMG Payor Initiatives services and quality improvement activities and initiates and/or provides recommendations for new and ongoing programs/projects related to clinical and service process improvement ensuring compliance with internal as well as external regulatory reporting agencies and accrediting bodies. Utilizing clinical judgment, careful listening, critical thinking, and assessment skills as well as excellent customer service skills the Clinical Quality Coordinator will collaborate with client partners, practice managers, team members, leaders, and external consumers to assure delivery of safe quality care.


The role is to be well versed in all phases of the continuum of care and clinical services delivery that views and values how all disciplines play an integral part of the system's overall strategic safety and quality plan. Researches questions and concerns to better understand processes, facilitate the service recovery process and recommend opportunities for continuous improvement. Participates as a member of multi-disciplinary teams that focus on clinical quality and service excellence. Identifies key educational topics based on quality reports and feedback from clients.


Experience in the development and expediting of educational plans and classroom facilitation. Has strong working knowledge of principles of adult learning and teaching methods and has formal or informal experience with leading adult education. Ability to assess the individual learning needs of a student and formulate a training plan.

  • Education: Bachelor's degree, required BSN. 
  • Experience: Minimum three years of experience in a health care or medical setting, required. Minimum two years of experience in clinical data collection and analysis, quality assessment, and improvement process, required.
  • Licensure/Certification/Registration: Current RN licensure in appropriate state, required. Certified Professional in Healthcare Quality CPHQ, preferred.

Effective analytical, problem-solving, and diplomacy skills. Ability to collaborate with other departments or disciplines to achieve the desired outcome. Ability to effectively communicate (both verbal and written) ideas to team members, management, and client base. The ability to organize tasks and resources and coordinate activities within multiple healthcare disciplines is essential. Proven analytical ability and organizational skills necessary to organize/assess information and provide recommendations based on data analysis. Working knowledge of regulatory requirements and governing standards, policies, and procedures. Monitors quality metrics and implements quality improvement plans. Reports issues/risks to leadership. Demonstrates knowledge of principles related to quality and performance improvement, current knowledge of JCAHO standards, applicable state(s) licensing requirements, and CMS standards. Understanding of process/quality improvement methodologies, health care statistics, and data collection and reporting. Demonstrates clinical expertise to effectively facilitate the evaluation of quality care and possesses advanced skills in data management as well as the ability to collect, analyze and present data to groups and individuals. Demonstrates ability to develop, organize, facilitate and lead special projects and has the ability to work collaboratively with physicians, clinical staff, and external organizations. Proficiency in Microsoft Office: Excel, PowerPoint, Access, Word and Project, and SharePoint. Proficiency in EPIC Ambulatory. Strong organizational and interpersonal skills. Excellent written and verbal communication skills. Detail and process-oriented with a focus on efficiency. Condenses complex information into a clear and precise clinical picture while working independently. Reports audit or clinical findings to appropriate staff or others in order to ensure appropriate outcome and/or follow-up for improvement as indicated. Business level math, basic statistical analysis. Ability to multi-task, prioritize, and effectively adapt to a fast-paced, changing environment. Travel to different practices and/or sites may be required. Ability to drive/travel to multiple locations/facilities as needed.
  • Additional Skills/Requirements (preferred): Telephone triage experience and familiarity with Schmitt-Thompson protocols are a plus.
