Novant Health is seeking a Pharmacy Tech II-Certified---Medication Listing Assistant, to join our hospital pharmacy team, to deliver optimal medication management across all dimensions of care, every time.
The Medication Listing Assistant is a certified Pharmacy Technician who supports pharmacy services by obtaining medication histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers. The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.
Works as part of a larger care team based outside the pharmacy, primarily in the Emergency Department
Schedule – 7on/7off schedule, 1130-2300, Wednesday - Tuesday
Minimum of 3 years retail pharmacy experience required
Hospital experience (in addition to retail experience) preferred

Essential functions include:
- Review, clarify, document and verify home medications with the patient or utilize alternate sources for verification such as patient’s family, caregiver, pharmacy or medical provider within 24 hours of admission
- Use high-level communication skills to perform a patient interview and obtain a complete home medication list.
- This includes contacting retail pharmacies or physician offices to verify the medication history with persistence to achieve the highest level of accuracy.
- Ensure complete and accurate documentation for each identified medication in the electronic medical record with careful attention to detail.
- Discuss any identified issues related to the medication history with the clinical pharmacist team to promote high-quality transitions of care.
- Filling and delivering medication orders for all age groups under the direct supervision of a pharmacist.
- Performing non-sterile compounding and repackaging.
- Answering and screening telephone calls and referring calls to a pharmacist when appropriate.
- Processing cart fills, refills to automated dispensing machines and replenishments to emergency kits and carts.
- Demonstrating proficiency with equipment and software.
- Collecting medication histories from patients and/or compounding sterile admixtures.

Novant Health is seeking a Clinical Pharmacist Operations Lead, to join our acute care pharmacy team, to deliver optimal medication management across all dimensions of care, every time.
The Operations Lead Pharmacist will perform all duties as outlined in the Pharmacist job description. In addition, the Lead Pharmacist will provide leadership to assigned Pharmacy staff by proactively assisting in high level clinical tasks, operations, training and education, and act as a liaison between staff and the leadership team.
Schedule – 1st shift, every 3rd weekend (1st shift – 0615- 1445 or 0700-1530)
-Residency training PGY1 or 2 years acute care experience
-Previous leadership experience preferred
-Board Certification required within 2 years of hire

- A Pharmacist is responsible for performance and/or supervision of various aspects of the medication use system including but not limited to: therapeutic recommendations, advocate for patient safety, preparation, distribution, monitoring, oversight of administration, and billing. This practitioner will be primarily based on the Operations Lead Team but will also rotate through other clinical pharmacy care teams of their interest.
- Leads and coordinates work activities for central pharmacy and provides feedback to team regarding performance under the direction of the leadership team.

- Triages issues to leadership and follows-up.

- Provides support for leadership team and demonstrates leadership for assigned practice area.
- Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
- Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,
- Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
- Makes appropriate evidence-based, patient- centered medication recommendations
- Participates in the management of medical emergencies
- Provides discharge medication review, reconciliation, and counseling as appropriate
- Maintains proficiency in and actively engages in central dispensing functions and operational programs
- Oversees pharmacy operations and technician practice/activities
- Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills
